Some documents require an Acknowledgement to be notarized, others require a Jurat. If notary language is not present indicating certificate type, you should question the party requiring the notarization on their preference.
Notaries are not allowed to select notary certificates for you.
A Notary Acknowledgement is a document certifying
-the signer personally appeared before the Notary on the date and county indicated
-the identity of the signer
-that the signer acknowledged executing the document
A Notary Jurat is a document certifying
-the signer personally appeared before the Notary on the date and county indicated
-the signer SIGNED IN THE PRESENCE OF THE NOTARY
-the Notary administered an oath or affirmation
-the identity of the signer.
